Insurance Billing
We are a provider for many insurance companies and can bill them directly in most cases. Please click on the following link to see which insurance companies we currently provide for. In some cases, a prior authorization is required by some carriers and the staff at Park Pharmacy will work diligently to provide all of the necessary paperwork and information needed to obtain approval.
In the event we are unable to bill your insurance carrier, please click on this link for a list of claim forms that can be completed and submitted for reimbursement from your carrier directly
Return Policy
Prescription medications are not eligible for return or refunds.
Vitamins, supplements and over-the-counter products are eligible for return/refunds with original receipt, in original packaging and condition up to 30 days from the date of purchase less cost of shipping.
